Referral Information for Healthcare, Social Services, and Criminal Justice Professionals

Programs Available

  1. Cynthia Day Family Center: Specialized residential treatment for individual/single women as well as pregnant and parenting women and their children.

  2. Recovery Housing: Safe and supportive housing for individuals transitioning from treatment to independent living.

  3. Medication Assisted Treatment: Combines counseling and medications for opioid or alcohol recovery.

  4. Mental Health Services: Therapy, counseling, and psychiatric care for emotional and behavioral health.

  5. Peer Recovery Support: Help from trained individuals with lived experience in recovery.

  6. Court Services: Support navigating legal systems and meeting court-related treatment requirement.

NOTE: We do not currently offer HIOP/PHP or IOP.

Patient Eligibility

  • Patients must be motivated to participate in recovery services.

  • Eligibility requirements vary by program and are tailored to meet individual needs.

  • Patients do not need insurance to be considered; Harbor Care offers a wide array of payment options, including Medicaid and most private insurance plans.

How to Refer

  1. Contact Harbor Care Intake Team:

    • Call 603-821-7788 or email hope@harborcarenh.org.

    • Provide basic patient information, including substance use history, medical needs, and the reason for referral.

  2. Submit Supporting Documentation:

    • Include any recent medical records, assessments, or treatment recommendations, including details that might impact treatment, such as housing stability and legal issues.

    • Provide any details on co-occurring conditions, medication management needs, or other relevant clinical information, including current mental health status.

  3. Coordinate Follow-Up:

    • Our intake team will connect with the patient to complete the enrollment process and coordinate an initial evaluation.

Harbor Care is distinguished in Patient-Centered Medical Home protocols, the HRSA National Health Service Corps, and HRSA’s Health Center Program. Harbor Care Health & Wellness Center is a Federally Qualified Health Center, a community-based health center that provides primary care and support services to underserved populations.
