Referral Information for Healthcare, Social Services, and Criminal Justice Professionals

Programs Available
Cynthia Day Family Center: Specialized residential treatment for individual/single women as well as pregnant and parenting women and their children.
Recovery Housing: Safe and supportive housing for individuals transitioning from treatment to independent living.
Medication Assisted Treatment: Combines counseling and medications for opioid or alcohol recovery.
Mental Health Services: Therapy, counseling, and psychiatric care for emotional and behavioral health.
Peer Recovery Support: Help from trained individuals with lived experience in recovery.
Court Services: Support navigating legal systems and meeting court-related treatment requirement.
NOTE: We do not currently offer HIOP/PHP or IOP.
Patient Eligibility
Patients must be motivated to participate in recovery services.
Eligibility requirements vary by program and are tailored to meet individual needs.
Patients do not need insurance to be considered; Harbor Care offers a wide array of payment options, including Medicaid and most private insurance plans.

How to Refer
Contact Harbor Care Intake Team:

Provide basic patient information, including substance use history, medical needs, and the reason for referral.
Submit Supporting Documentation:
Include any recent medical records, assessments, or treatment recommendations, including details that might impact treatment, such as housing stability and legal issues.
Provide any details on co-occurring conditions, medication management needs, or other relevant clinical information, including current mental health status.
Coordinate Follow-Up:
Our intake team will connect with the patient to complete the enrollment process and coordinate an initial evaluation.
Harbor Care is distinguished in Patient-Centered Medical Home protocols, the HRSA National Health Service Corps, and HRSA’s Health Center Program. Harbor Care Health & Wellness Center is a Federally Qualified Health Center, a community-based health center that provides primary care and support services to underserved populations.
